I was putting insulin into the cartridge but some of it sprayed out the other way, should i be concerned?

I would use a different cartridge and contact the company about it spraying out.

Yes that shouldn’t happen and likely means that the vial is cracked or isn’t sealed properly where the plunger is located. I would discard it because it will be nearly impossible to know if the dosing is accurate and precise.
